State-of-the-Art Laboratory including CBC, Chemistry & Lipid Panels as well as inflammatory markers and complete hormonal panels. Lab fees are usually covered by patients insurance.

** Custom Hormone Panel fees are available through Age Management Institute (Drawn only through Lab Corp), for an additional fee of $550.00, which represents an 80% discount off the retail cost, if there is no private insurance coverage available.

AMI COMPREHENSIVE HEALTH PROGRAM
$1500 Per Year Which Includes:

• Full hormonal optimization and complex medical management of medical and age related diseases.
• Preferred and timely access to AMI for office follow-up, phone and email contact.
• Preferred hormonal, pharmaceutical and supplement pricing.
• Medical specialty referral service.
• Full access to all AMI medical, dietary and supplement education and services.
• The Ultimate Anti-Aging Program
• Limited primary physician care. This service does does not include 24 hour on-call coverage for emergencies or hospitalization.

AMI LIMITED HEALTH PROGRAM
$1200 Per Year Includes:

• Partial hormone optimization
• All other services as detailed in the AMI Comprehensive Program

AMI CONSULTATION ONLY PROGRAM
$800 Per Year

• This program is designed for patients who require partial hormonal optimization and have a primary care provider who will be responsible who will be responsible for age related screening and primary medical care.
• Preferred hormonal, pharmaceutical and supplement pricing.
• Twice yearly AMI office visits.

** AMI will bill annually for Health Program fees. Our Program Professional Fees are meant to be cost and modest, as well as to provide either complete or partial Anti-Aging Programs that are traditionally only available to the most affluent.
